Drug therapy in brain ischaemia.

چکیده
After many years of research, drug treatment in cerebral hypoxia is still controversial and no treatment appears to be firmly established. Not even the mechanisms causing damage have been established (Siesjo, 1981), only that they are linked to a disturbance in the oxygen demand: supply ratio. Enthusiasm for new modes of therapy have waxed and waned. We have witnessed alternating enthusiasm for cerebral vasodilators, cerebral vasoconstrictors, cerebral metabolic depressants, free radical scavengers, membrane stabilizers, calcium blockers, drugs that improve blood rheology, and drugs that are reported simply to improve cerebral function in a "black box" fashion. Part of the apparent conflicts in the literature may result from great variety in the models used to test pharmacological brain protection. To clarify some of the confusion it is important to differentiate between species, preand post-treatment and the main categories of cerebral hypoxia (table I).

Commentary: New View on Treatment of Drug Dependence
In the 1960s, discovery of pleasure system (defined as reward system) in the brain that may underlie drug reward and addiction encouraged many scientists to investigate the mechanisms by which drug abuse affects central nervous system function. In this regard, investigators developed several drugs targeting the brain reward system for drug dependence therapy. However, no positive results obtain...
متن کاملAcidotoxicity in brain ischaemia.
Intracellular calcium toxicity remains the central feature in the pathophysiology of ischaemic cell death in brain. Glutamate-gated channels have been thought to be the major sites of ischaemia-induced toxic calcium entry, but the failure of glutamate antagonists in clinical trials has suggested that glutamate-independent mechanisms of calcium entry during ischaemia must exist and may prove cen...
متن کاملTargeting the SUMO pathway for neuroprotection in brain ischaemia
Small ubiquitin-like modifier (SUMO) conjugation (SUMOylation) is a post-translational protein modification that modulates almost all major cellular processes, and has been implicated in many human diseases. A growing body of evidence from in vitro and in vivo studies demonstrates that increasing global levels of SUMO conjugated proteins (global SUMOylation) protects cells against ischaemia-ind...
متن کاملExploring the role of dimethylarginine dimethylaminohydrolase-mediated reduction in tissue asymmetrical dimethylarginine levels in cardio-protective mechanism of ischaemic postconditioning in rats
Objective(s): Reperfusion of ischaemic myocardium results in reduced nitric oxide (NO) biosynthesis by endothelial nitric oxide synthase (eNOS) leading to endothelial dysfunction and subsequent tissue damage. Impaired NO biosynthesis may be partly due to increased levels of asymmetrical dimethylarginine (ADMA), an endogenous inhibitor of eNOS. As dimethylarginine dimet...
متن کاملCombination therapy of rosiglitazone, a peroxisome proliferator-activated receptor-gamma ligand, and NMDA receptor antagonist (MK-801) on experimental embolic stroke in rats.
Peroxisome proliferator-activated receptor-gamma (PPAR-gamma) agonists have been found to have potent anti-inflammatory actions and suggested as potential therapies for brain ischaemia. Glutamate is the most common excitatory neurotransmitter in the central nervous system and is released excessively during ischaemia. Stroke therapy will require combinations of drug classes, because no single dr...
متن کاملBiochemical markers of cerebral damage.
Neuroprotective therapy is likely to be most effective in preventing or minimizing the effects of cerebral ischaemia when given as early as possible after the insult. Neuropsychological testing is the gold standard for assessing minor cerebral damage, but is carried out several days or weeks after surgery and is too late to be useful for instituting therapy. Current intraoperative cerebral moni...
